Tutor/Student Progress Report

SCHOOL LITERACY PROGRAM

Thank you for taking the time to fill out this report. Please
answer the following questions, giving examples as needed.

	Did your student honor his/her commitments, i.e. coming on time, being respectful, attending on a regular basis, willing to work?
	What progress did you observe in your student? In reading?
	In writing?
	In attitude/behaviour?
	Is there anything you found difficult/problematic when tutoring your student?
	What else would you like to see included in next year's tutor training?
	What materials/themes in the Oasis were of most use to you and your student?
	What did one of your typical lessons entail?
	Did you try to make your lessons relevant to your student's interests/needs? If yes, how so?
	Can you give an example of a particularly rewarding or interesting experience you had with your student?
	Did the 20/15 program and the incentives help you? The juices and snacks? Why or why not?
	Do you use a computer (including Internet/reading/ writing software, literacy sites, creating Word Documents, etc) at the school or at home?
	Are there any other materials you would like Literacy Unlimited or the school to add to the School Literacy Program?
